We can't wait to meet you!The RN or LPN is responsible to the Unit Manager and its clients for rendering nursing care in assigned area in terms of individualized customer needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse.
Essential Duties and Responsibilities 1.
Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned customer/family as evidenced by documentation and observation of positive customer care outcomes2.
Initiates and leads team conferences in development of individualized nursing care plans3.
Assists with or institutes emergency measures for sudden adverse developments in customers4.
Contribute to the development of the customer/significant other plan of care based on the identified customer problems/needs5.
Carry out physician orders accurately, directly and indirectly6.
Assist in the evaluation of the effectiveness of the nursing care administered relative to the customer/ significant other identified problem/needs7.
Document pertinent observation and intervention as appropriate in the EMR8.
Perform selected tasks related to customer care and the customer's environment in a safe manner9.
Administer medication accurately, observing customer response, as evidenced by documentation in the EMR and lack of negative outcomes10.
Perform limited I.
V.
Therapy procedures on those customers receiving peripherally inserted Intravenous fluids, pursuant to LPN scope of practice.

Maintain and coordinate communication among customer/significant other, team members, and physician as evidenced within the EMR, client, and health care team feedback.

Required Education and Experience High School Diploma or Equivalent Graduate of an accredited LPN/RN program Required License/Certification Valid State LPN/RN license Current CPR certification Preferred Education and Experience A minimum of one (1) year of nursing experience in a SNF
